05-12-1989 Special Called Meeting r-- ., ~ , l i ~,1 I, AIRPORT AUTHbRITY MEETING SPECIAL CALLED MEETING MAY 12, 1989 Chairman Good called to order the special called meeting of the Airport Authority at 11:45 AM, Friday, May 12, 1989. Those members present were Mr. David Good, Ms. viki Brigham, Mr. Blaine Price and Mr. John Dennis. The special called meeting was held followin~ a Pre-construction Conference. Attached to these minutes w1ll be included the minutes from wilber smith & Associates of the Pre-construction conference that was held at 10:30 AM. Chairman Good ~resented two resolutions for the Authority to sign. Resolut1on #1 is a contract is between the Department of Transportation, Federal Aviation Administration and the Airport Authority. Resolution #2 is a contract is between the Airport Authority and Haygood Contractors Incorporated. A motion was made by Ms. Brigham, seconded by Mr. Dennis and was unanimously adopted to approve Resolution #1 as presented. RESOLUTIONS OF PEACHTREE CITY AIRPORT AUTHORITY RESOLVED, that the Peachtree city Airport Authority is hereby authorized to contract with the Georgia Department of Transportation, as well as any other state agencies, with respect to construction of the new runway at Falcon Field Airport, F'\ Peachtree city, Georgia, FAA AlP Project No. 03-13-0146-03, as well as all improvements related thereto, and any other improve- ments to Falcon Field Airport, including, but not limited to, clearing and grading, paving, painting, lighting, constructing instrument approach facilities, constructing taxiways, ramps and other facilities, and any other activities incidental thereto; that the Authority is further authorized to contract with the Georgia Department of Transportation with respect to the issuance of a grant of state funds for partial or total completion of this proj ect; and further that David M. Contractor must have superintendent on the job at all times. 3. Notice to Proceed will be issued Monday afternoon, May 15, 1989. The Contractor delivered insurance certificates. Bonds will be delivered Monday morning, May 15. Schedule will begin ten days after Notice to Proceed or as soon as the Contractor begins work. Two hundred fifty (250) days are allowed for completion of the job. Reviewed Safety Plan - NOTAM's will be issued as necessary. Contractor must contact F.B.O. Install erosion control measures immediately. Maintain throughout the job. DBE contractor has been approved. Must fiie report at end of the job. Pay request will cut off on the 25th day of the month. Project superintendent will submit request to the resident inspector. Copies will be, sent to WSA and GA DOT - William E. Mullis, 1001 Highway 19 South, Griffin, GA 30223. All change orders must be approved by FAA before work is initiated. Submit shop drawings to WSA or resident inspector. Burning will be allowed on-site but controlled depending on conditions. Weather condi- tions will determine when burning is allowed. Maintain haul roads at, all times. No equipment allowed on runways or safety areas without notice to F.B.O. (Mr. Jim Holder _ Aerospec). Notify the resident inspector if it is necessary to be on the runway. Stockpiles and staging area should be out of the safety area. Liquidated damages are $525 per day to cover cost of inspection and engineering. A $500 penalty will be charged for cutting FAA cable. Review plans for locations. 15. Stay out of the terminal area. Contact the resident inspector prior to grading area around the hangar area. 16. Resident Inspector will locate sanitary sewer as they are known. Change orders must be approved in advance of work. 2. FAA must receive weekly progress schedules. 3. Testing must be done by the specifications. Notify the resident inspector immediately about test failures. Deficiencies in the plans or construction will not be paid for by the FAA. 4. Daily logs will be kept. Copies of the weekly reports will be sent to the State and FAA. 5. Safety: a. Set limits in the field b. Backfill ditches and trenches quickly c. Use displaced threshold as outlined in the safety plan d. Control smoke and dust 6. Contractor is required to review labor and safety brochures. EEO statements are required in all subcontractors contracts. Wage rates must be posted. Employees will be interviewed for wage cbmpliance. FAA must be notified if any job classification has not been listed. ~.. ; 7. Submit weekly payroll statements to the resident inspector within one week after payment. Barry Amos, Peachtree City Engineer, stated that the contractors must obtain permits for construction, land disturbance, signs and trailers prior to construction. Review with him the requirements for on-site burning. Stan Neeiy of PCDC restated that access roads must be maintained. He questioned if all easements are to be cleared, which they are. Questions were asked about the following: 1. Location of construction trailers. Reed Bull suggested off the nc 1h end access road. Water plant operation has key to gate. Coordinate with water plant. 2. Survey points to be used regularly should not be on the runway or safety area. 3. He is aiso responsible for coordinating field testing and laboratory work. Contractor shall have a competent superintendent on the job during all phases of construction. Engineer's Inspector will not superintend the Contractors men or his work. PROGRESS SCHEDULE:Contractors are required to submit a proposed schedule of construction activities within 10 days of the Notice to Proceed. SAFETY & SECURITY: Contractor is responsible for maintaining safety and security of job site. See Safety Plan and Special Provisions ENVIRONMENTAL: Erosion control devices as shown on drawings shall be installed as early as possible and shall be maintained throughout the life of the project. LABOR REQUIREMENTS:Contractor and his subcontractors must comply with Section 100 of the Wage and Labor provisions of specifications. Submit on weekly basis payroll records to the Engineer. FAA specifications will be utilized, Contract time is 250 days commencing on the first day of work but within 10 days aher Notice to Proceed. CONTRACT STATUS AND NOTICE TO PROCEED: The Contracts have been received and are being reviewed by the Airport Authority. A Notice to Proceed will be issued as soon as contracts are signed and funding is approved by the Georgia Bureau of Aero- nautics.
